Giant papillae (>1 mm) are characteristic of vernal keratoconjunctivitis and GPC. **Key associations:** Itching, ropy/mucoid discharge, and contact lens or atopy history. ===== Follicular Reaction ===== **Morphology:** Rounded, translucent-to-grayish, avascular elevations with vessels coursing around (not through) them, representing subepithelial lymphoid germinal centers; most prominent in the inferior fornix/lower tarsus (and characteristically upper tarsus in trachoma). **Histology:** Lymphoid follicles with germinal centers — a lymphocytic response typical of viral and chlamydial infection and drug toxicity. **Typical causes:** Viral (adenovirus most common), chlamydial (inclusion conjunctivitis and trachoma), toxic/medicamentosa (e.g., topical drug reactions), and Parinaud oculoglandular syndrome (e.g., Bartonella). **Key associations:** Watery discharge, preauricular lymphadenopathy, and recent upper respiratory infection or sexual/STI history. ===== Causes by Reaction Pattern ===== ^ Category ^ Reaction Pattern ^ Specific Causes ^ | Allergic | Papillary | Seasonal/perennial allergic conjunctivitis, vernal keratoconjunctivitis, atopic keratoconjunctivitis | | Mechanical/Immunologic | Papillary | Giant papillary/contact lens–induced conjunctivitis (lens deposits), ocular prosthesis, exposed sutures | | Infectious | Papillary | Chronic bacterial conjunctivitis (S. aureus, S. pneumoniae, H. influenzae, Moraxella) | | Viral | Follicular | Adenovirus (EKC, pharyngoconjunctival fever, nonspecific follicular conjunctivitis), other viruses | | Chlamydial | Follicular | Inclusion conjunctivitis (C. trachomatis D–K), trachoma (serotypes A–C) | | Toxic/Medicamentosa | Follicular | Chronic topical medication toxicity (e.g., antivirals, glaucoma drops, preservatives) | | Granulomatous | Follicular | Parinaud oculoglandular syndrome (Bartonella henselae, tularemia, sporotrichosis) | ===== Clinical crossover ===== Some conditions produce a mixed picture — trachoma, for example, classically shows follicles but develops an intense papillary component when inflammation is florid. ===== References ===== - [[https://pubmed.ncbi.nlm.nih.gov/32339665/|Kenny SE, Tye CB, Johnson DA, Kheirkhah A.
